[File:Line] Clarification needed ``` ## Guidelines - Be specific — reference exact file and line numbers - Explain WHY, not just WHAT is wrong - Always include at least one positive comment - Suggest fixes, don't just point out problems - Distinguish between blockers and nice-to-haves
